Identify a clinical problem or issue that you want to explore pertaining to advanced practice nursing care. The clinical problem or issue should apply current evidence-based practice to improve patient outcomes in an acute care setting. The clinical problem should focus on the acute care setting and relate to a clinical patient or patients with a cardiovascular (Heart failure) disorder from the AG-ACNP perspective.
Problem:
There are too much HF education and to little follow up once a patient is discharge from the hospital. About half of the HF patient population will die in the next 5 years and about 30 % of them is readmitted to the hospital in 30 days to 6 months. What to do to stop them? Is home based care the key to have them stay home with home healthcare services becoming specialized in HF?

Select resources which focus on and which will strongly support improving patient outcomes. Select upto 12 qualitative or quantitative peer-reviewed primary research articles published within the last 5 years. You may use articles from both peer-reviewed nursing-specific and health care journals; however, a minimum of half of the articles selected must originate from a peer-reviewed nursing journal.
2.    Summary: In your own words, write a summary of three to five sentences and/or an evaluation of each of the sources in terms of how each source you have selected will assist you in organizing, developing, and presenting your final evidence-based paper. Each source summary should mention the thesis of the work and how this work is important for your argument within the overall paper.

The story of Henrietta Lacks implicates the medical community in treating Lacks and her family as abstractions more than as human beings.     1.Track the human consequences of the medical community having treated Lacks and her family as abstractions more than as human beings.
