Congest heart failure patient and the readmission in 30 days. What we can do to reduce the readmissions and prolong life out of the hospital?
The mortality and readmission rates of congestive heart failure (CHF) patients in the United States and the world in general, have caused alarm and need to be addressed as an urgent matter. According to Gosselin et al. (2014), in an observation supported by Banoff et al. (2016), about fifty percent of the people suffering from CHF dies within five years after being diagnosis while about thirty percent get readmitted to the hospital in 30-180 days after discharge. Such readmissions and deaths are very costly to both the government, hospitals and the individual families involved. Specifically, the readmissions come with extra costs to the hospitals, and patients might not make it out of the hospitals. This in its turn have been addressed by the government and its regulation departments, The Medicaid and Medicare. To address this issue, the current proposal recommends educating patients and healthcare providers on best practices towards reducing readmission rates, hence reducing healthcare cost. If the patient is unable to understand teaching, the patient’s relatives can be taught on how to care for the patient at home as well as how to recognize signs and symptoms when it is time to contact the primary care physician. Health workers will be taught on doing follow up care over the phone and physically, assessing patients comprehensively to rule out unnecessary readmissions, and teaching relatives on how to care for the patients.
