Heart failure patient need to be educated in a simpler way. This project is done to show a way to help reduce the heart failure patient’s readmissions. Existing heart failure help and education is available upon discharge but it needs to be simplified. Simpler ways of educating the patients is needed because there is too much information available to patients. Most of the patients goes home and does not follow the discharge instructions due to either lack of understanding or overwhelmed with too much information. To involve the physicians' offices and to get a better communication between hospitals, the physician's office, and the community centers/ health department, it will improve the outcome for the patient's life style changes. The solution suggested is to take the existing heart failure education and simplify it with lemans term for better patient understanding and to add a 30 minute education class in the physician's office or community centers/ health department.

Methods Used to Evaluate the Effectiveness of the Proposed Solution
The selected groups of patients to be used in this project must be ready to cooperate with the nurses and the entire medical team that will be involved. On the other hand, the medical staff ought to embrace the plan to the extreme end for the outcome to turn out positively. Just to mention, cohesion amongst the participating teams would be crucial in the achievement of the desired success. The proposed solution to the reduction of heart failure disease and readmission of patients will be evaluated through divergent methods. As the proposal entails, the twenty patients who will be subjected to the current method of education will be separated from the other twenty who be educated using the simplified method. The fact that the death rate of HF patients is on the rise despite the introduction of more advanced techniques reflect the need to embrace other measures that can fetch the results that are quested for (Arthur et al, 2015).
The outcome will be evaluated using the pie chart where patients who will be readmitted within the first thirty days will be categorized basing from the different groups. A deeper analysis reveals that people suffering from this disease have different levels of understanding and educations. Consequently, the process will be smooth by the use of the simplified tools.
