The clinical problem or issue should apply current evidence-based practice to improve patient outcomes in an acute care setting. The clinical problem should focus on the acute care setting and relate to a clinical patient or patients with a cardiovascular (Heart failure) disorder from the AG-ACNP perspective.
Problem:
There are too much HF education and to little follow up once a patient is discharge from the hospital. About half of the HF patient population will die in the next 5 years and about 30 % of them is readmitted to the hospital in 30 days to 6 months. What to do to stop them? Is home based care the key to have them stay home with home healthcare services becoming specialized in HF?
