  •             The aim of the case management is to enhance effective medical care on a patient during the first examination. Case management poses as the appropriate intervention when dealing with a patient for the first time because it indicates how to conduct a diagnosis. Moreover, through the case management, it is possible to change and update a diagnosis accordingly. The purpose of the case management is to design a process that will enhance effective diagnosis and provide the direction on the next step (Cohen & Cesta, 2005). After diagnosing the patient, the next step is to set up a reasonable plan on the appropriate medication and treatment to provide. The case management provides guideline on what to perform as a medical profession in order to be sure of about patient’s problem.
  • OBJECTIVE:
  •             One of the significance aspects of employing the case management plan is to conduct a tentative diagnosis for the client. The following are various issues to consider when conducting the tentative diagnosis.
  • Crisis Service
  •             There is a need to employ crisis services in order to provide the required necessary services at the right time. The other importance of a crisis service is to provide the necessary aid through improving the performance in dealing with various situations that can possibly turn into crises. Moreover, a crisis service helps in making the right decision about which services to employ and can work best concerning a certain situation. Through crisis services, it is possible to contact with potential clients in case of crisis occurrence and offer the required assistance (Thomas & Hersen, 2010).

After making the proper introductions, the next objective is to enquire the history of the client in relation to the potential problem. In this case, the first option is to establish the chief complaints of the client. After that, the next issue to consider is history of the present illness. Then the next issue is to establish the current health status and enquire whether the client has been getting any treatment. Moreover, it is necessary to identify certain data concerning the client. This means getting to know the age, date of birth, sex and race of the client (Cohen & Cesta, 2005).
  • Physical Exam
  •            This is one of the excellent ways to gather information regarding the problem affecting the patient. The importance of conducting physical assessment is that a medical profession gets to know the baseline of information ought to help in future assessments. Moreover, physical assessment assists in validating all the complaints made by the client when conducting the history sessions. Physical assessment also assists to monitor the health problems made by the client.   • Diagnostic Tests
  •             A diagnosis entails conducting various tests in order to come up with a comprehensive conclusion of the best treatment to give. Provide the best treatment means giving the right medication after understanding what has caused the diseases and the possible future response. While offering treatment, it is possible to decide whether to give various medicines or there is a need for the client to take several doses while still in hospital until the situation changes.
  • Roles of the case manager
  •             Some of the roles as a case manager include enhancing that every client receives the right services and proper medication. My role as a case manager is to make sure there is a reduction in number of crises that may occur in future. Moreover, through enhancing proper management, it will be possible to conduct earlier detection of various problems effecting clients (Kathol, Perez & Cohen, 2010).
